Feel better in the comfort of your own home. I specialize in care management assistance to an array of individuals. Whether the client needs daily or weekly assistance due to aging, illness, recovery, or rehabilitation, I will provide an individualized service that you can trust.
I understand that not one care plan fits all. Services can include anything from caregiver and client coaching; follow up on medical regimens; monitoring through phone calls and note taking; and in person check-ins. I will take the time to get to know the client and their family and develop an individualized care plan that fits the client's specific needs.
I'm a Bainbridge based care manager working for elders in the Kitsap area since 2007. I completed undergraduate work in social and human services and counseling in 2008 and I earned my Care Management Certificate in 2015 from the University of Washington. I have developed the knowledge and skills necessary to meet the needs of adults and their families facing challenges related to aging and illness through continuing education and consultation from other professionals. My long-term community involvement has created relationships with local services and care providers. I get deep satisfaction from supporting families and individuals through transitions related to aging while working with them reach the best possible outcome.
Finding the right local services can be difficult. I have experience working with various local services and providers tailored to aging care, and can help the client locate and use them.
Communicating effectively with a doctor helps to prevent confusion and ensure proper care is being received. I have experience keeping track of symptoms and medical events and relaying that information to the client's doctor to help ensure the best care.
I have experience navigating care facility admittance, and I can help the client to smoothly transition either in or out of a care facility without them having to stress over the details.
Open communication is important to keep the family up to date with the clients needs and work with them to develop a plan of action. I keep in communication with the family of the client to help support them and keep them up-to-date on the status of the client.
When keeping track of important events and appointments is difficult, I help to create and maintain a calendar of the clients schedule to help avoid missed appointments.
I coordinate with the client's doctors and nurses to prepare and maintain a medication schedule, keeping medications organized, secure, and properly dosed.
The clients safety and standard of care are of the utmost importance. I help develop a system specific to the clients needs that outlines all the necessary information for proper care.
Some problems have early warning signs. I have experience detecting problems and crises for the client and enacting preventative measures before they become insurmountable.
In the event that a crisis is unavoidable, I have experience calmly and effectively guiding the client and their family through the crisis and help them to establish normalcy after the crisis has passed.
Caregivers can sometimes require guidance on the procedure specific to the client. I work with the client's caregivers, old and new, to guide them on proper protocol and keep a high standard of care for the client.
Homes require regular upkeep and maintenance. I help coordinate with service providers to keep up with the maintenance of the home.
I have many years of experience helping families navigate grief, loss, major life transitions, and end of life care. I offer consultation services to help determine what the best course of action is for the client, and inform families on potential next steps for the care of their loved one.
"Ariana has been a tremendous help to us as we negotiate the minefield that is eldercare. Nothing really prepares you for helping your parents with elderly health care and long-term care issues. Ariana's advice and counsel has made our journey much easier and less painful for us and our parents."

“Ariana provided a range of exceptional caregiving services to both of my parents, and to my mother-in-law, beginning in the late 1990’s. That care continued until my mother-in-law’s passing in 2016. Ariana is expert at what she does and one of the kindest, most compassionate people I’ve ever met. My parents and my mother-in-law adored her and loved having her around. My wife and I, as well as my siblings, are in awe of Ariana's abilities, and deeply grateful that we connected with her and that she was able to help us through the years as our parents’ health declined. I wholeheartedly recommend Ariana and her team.”
"Ariana began support services for my grandparents in the late nineties. She was continually and consistently beside our extended family through my grandmothers passing and years later my grandfathers. After 20 years of caring support for my grandparents she is part of the extended family. I cannot recommend her and her team highly enough."
Let me know about your questions or concerns and I will be happy to get back to you.
