Feel better in the comfort of your own home. I specialize in care management assistance for an array of individuals. Whether the client needs daily or weekly assistance due to aging, illness, recovery, or rehabilitation, I will provide an individualized service that you can trust.
I understand that one care plan does not fit all. Services can include anything from caregiver and client coaching; follow up on medical regimens; monitoring through phone calls; note taking; and in person check-ins. I will take the time to get to know the client and their family so that I can develop an individualized care plan that fits their specific needs.
I'm a Bainbridge based care manager working for elders in the Kitsap area since 2007. I completed undergraduate classes in social and human services and counseling in 2008 and I earned my Care Management Certificate in 2015 from the University of Washington. I have developed the knowledge and skills necessary to meet the needs of adults and their families facing challenges related to aging and illness through continuing education and consultation from other professionals. My long-term community involvement has created close relationships with local services and care providers. My mission is to support families and individuals through transitions related to aging while working with them to reach the best possible outcome.
Finding the right local services can be difficult. I have experience working with various local services and providers tailored to aging care, and can help the client locate and use them.
Communicating effectively with medical providers helps to prevent confusion and ensure proper care. I can create systems for tracking and communicating symptoms and practices at home. Getting accurate and timely information to providers helps them do their jobs.
I have experience navigating care facility admittance, and I can help the client to smoothly transition either in or out of a care facility without them having to stress over the details.
Open communication is important to keep the family up to date with the clients needs and work with them to develop a plan of action. I keep in communication with the family of the client to help support them and keep them up-to-date on the status of the client.
When keeping track of important events and appointments is difficult, I help to create and maintain a calendar of the clients schedule to help avoid missed appointments.
I coordinate with the client's doctors and nurses to prepare and maintain a medication schedule, keeping medications organized, secure, and properly dosed.
The clients safety and standard of care are of the utmost importance. I help develop a system specific to the clients needs that outlines all the necessary information for proper care.
Some problems have early warning signs. I have experience detecting problems and crises for the client and enacting preventative measures before they become insurmountable.
In the event that a crisis is unavoidable, I have experience calmly and effectively guiding the client and their family through the crisis and help them to establish normalcy after the crisis has passed.
I work with the client's caregivers, old and new, to troubleshoot their concerns, guide them on proper protocol and keep a high standard of care.
Homes require regular upkeep and maintenance. I help coordinate with service providers to keep up with the maintenance of the home.
I have many years of experience helping families navigate grief, loss, major life transitions, and end of life care. I offer consultative services to help determine what the best course of action is for the client and how family and friends can support.
